Once it has been determined that Dr. Howard Tung is the specialist for you, please call to make an appointment. Once it has been confirmed you can download the New Patient forms. We suggest you fill them out in the comfort of your home and bring them with you to your first visit. These forms are not fillable.
On the day of your visit
Please bring the forms filled and signed. Also, make sure you bring your Driving License or any other mean of identification, insurance card and most importantly your radiological images (MRI, X-Rays, CT scans, etc) with their reports. These images can be brought in CD form or films. They can also be seen in some web sites. If you do not have images please call us, we will try to help you locate them. We will NOT be able to see you without the above documents and a new appointment will be offered.
We strive to make our appointments available and timely for you. Please help us by arriving 15 minutes early to check in and complete any paperwork or updates we may need.
We appreciate your understanding that our physicians occasionally fall behind schedule due to emergency surgeries or complicated cases. We will keep you informed of our schedule upon arrival if we are running behind.
Payments
Payments and co-payments are due at the time of visit.
Insurance Billing
Dr. Tung is contracted with a wide variety of insurance plans including Medicare, most PPO insurance plans and Worker’s Compensation. To ensure the most up-to-date in-network information, please call your health insurance plan or visit their web site to view their current providers. If you are a new patient looking for care, please contact our office to discuss your insurance before your appointment.
Prescription Requests
For prescription refills, please contact your pharmacy. They will contact us electronically for your request. Please allow a minimum of 24 hours for all prescription refills.
